Type of site required.
Single sites are 3m x 3m. Double sites are 6m x 3m.

If you have a van or trailer please provide space requirements. If you require more than a 3m x 3m space you will need to select a DOUBLE site.

If you selected a NON-POWERED site please write NA in this box. If you selected a POWERED site, please fill in the text box below with the following details: Electrical Appliance, Electrical Power Required (amps) and Electrical Power Output (Watts). You will ONLY be supplied with the requirements you list on this form. There will be no extra outlets on the day. MAXIMUM of TWO outlets per market stall - additional outlets will incur a fee. You must supply your own leads and they must be tested and tagged.

Please note: Your insurance policy must be up to date with a minimum of $10,000,000 indemnities. If your policy expires before the event date, please attach your current policy document for the application and ensure a copy of the renewal is forwarded to the Festivals team before event day.
Declaration
I have received, read and understand the below conditions for the 2017 Mackay Regional Festival of Arts - G & S Engineering Wine & Food Day. I further agree to indemnify the Mackay Regional Council against any claim whatsoever that may be brought against Mackay Regional Council by any person in relation to this market stallholder agreement.

All site payments must be paid at time of application.  All site applications are subject to event organisers discretion.

You will be issued with a receipt as proof of payment. This receipt will be sent to the postal address indicated on your application form.

CONDITIONS

The following conditions have been implemented to present a quality event and service for the benefit of the community and our stallholders and food vendors. Should you require any assistance in meeting any of the following criteria, please contact the Festival and Events Producer on 07 4961 9763.

  1. Food handling facilities and service must comply with the Mackay Regional Council Health and Regulatory Service Division. A copy of your Temporary Food Permit or License must be supplied to the Festival and Events Producer prior to the event along with your application.
  2. ALL CARS ARE TO BE REMOVED PRIOR TO EVENT START.
  3. Food Vendors must have attended a Food Handling Information Session at Mackay Regional Council in the past 12months. If not you must attend a session prior to the Festival 2018.
  4. All food vendors and market stallholders must hold a current public liability insurance policy with a minimum cover required for their specific products or services
  5. All food vendors and market stallholders must supply their own marquee (where applicable) and materials.
  6. Equipment to secure marquees must be supplied by the food vendor or market stallholder (eg. Sand bags, weights etc.)
  7. Food vendors and market stallholders are responsible for the bulk rubbish removal from their allocated site area - cleaning labour fees will be charged to the food vendor or market stallholder upon leaving rubbish on site.
  8. Food vendors and market stallholders are liable for restoration claims for any damage they may cause to the site areas.
  9. Failure to comply with all conditions may lead to withdrawal of your site.
  10. Food vendors and market stallholders may be filmed or photographed during the event and agree for footage and images to be used for promotional use by Mackay Regional Council.
  11. Application and payment must be completed by 18 MAY 2018.
  12. Site payment refunds will only be given if the event is cancelled.
